You may also designate an authorized agent to make a request on your behalf. Authorized agents must submit written proof that they are authorized to act on your behalf. We may deny requests from authorized agents who do not submit proof that they have been authorized by you to act on their behalf.

Access Health reserves the right to modify or update this Privacy Policy at any time and without prior notice to you, effective upon posting of an updated version of Privacy Policy as identified by the “Effective” date at the beginning of this Privacy Policy.
